Complete registration before your procedure
Pre-registration helps SPOSC collect the demographic, insurance and health-history information needed to prepare for your visit. Complete all requests as soon as they are received.
- Follow the registration instructions in your patient-specific invitation.
- Confirm your legal name, date of birth, address and contact information.
- Provide requested insurance information and supporting documents as directed.
- Provide an accurate medication, allergy and health-history list.
- Respond promptly if the registration or clinical team requests additional information.
Your SIS Exchange invitation: When your case is scheduled, an invitation is automatically sent by email or text message. Follow the secure, patient-specific link you receive. If you have not received an invitation or need a replacement, contact SPOSC at (910) 725-6740; staff can resend it.
